Clinical Pharmacist
CurrentRepeatedly called upon to help manage complex, demanding customer relationships with hospital coordinators. Demonstrates reliability, flexibility, and emotional intelligence and is sought after by other staff members as “go to” person when issues arise. Mentors pharmacists and technicians in clinical management of patients on IV therapies. Routine telephone communication with physicians, nurses, patients, and caregivers to create patient specific care plans while on IV therapy. Routine interaction with prescribers, liaisons, and VNA’s regarding drug interactions, dosage adjustments, order clarification. Supports billing department with clinical rationale regarding reimbursement issues. Interacts with prescribers with recommendations to mitigate recent supply issues of many nationally unavailable IV medications. Determines drug stability, compatibility, and any necessary calculations for intravenous med orders including antibiotics, chemotherapy, hydration, inotropes, pain management, and parenteral nutrition. Performs ongoing laboratory monitoring and assessments for patients on IV antibiotics (including aminoglycosides and vancomycin), chemotherapy, hydration, inotropes, pain management, and parenteral nutrition with recommendations to prescriber when warranted based on clinical presentation of patient.
