Current-Assist in development of outreach plan in conjunction with SSVF team to enroll families in services-Assist clients in securing housing, basic needs, medical and mental health services including substance abuse treatment when necessary, job training access, and public benefits where eligible with a focus on community reintegration-Work to effectively meet clients' needs and resolve individual barriers through follow-up, advocacy and collaboration with Front Steps staff and other community service providers-Maintain accurate and timely records of activities, case management notes, and services provided to each client using HMIS-Ability to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with effective record keeping-Develop and maintain relationships with clients, staff and local service providers, participate in community meetings pertaining to services benefitting clients, and serve as a representative of Front Steps at community events-Provide regular, written reports to the SSVF Program Manager and additional reports as assignedOther duties as assigned to support the mission of the organization-Assess housing barriers of female Veterans and families experiencing homelessness to determine housing and service needs-Provide mediation and advocacy with landlords on the client's behalf to develop a plan to obtain and or maintain housing-Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies, debtors and creditors)
