I observe patient behavior and activity for 12 hour periods. During this time I am responsible for documenting pertinent information as well as relaying that information to doctors, nurses, nursing assistants, and fellow patient safety attendants. The typical patient population that I work with include fall risk, involuntary admissions, suicidal, demented/cognitively impaired, psychiatric, rehab, and DSS. The primary focus is my patient and the primary objective is to maintain patient safety at all times.

This facility is representative of my third clinical affiliation and is an inpatient rehabilitation facility at CMC-Main. The demographics of my patient population are inclusive of oncology, CVA, SCI, GBS, TBI, COPD, acute respiratory failure, and debility. I was given the opportunity to participate in therapy grand rounds and huddles with fellow clinicians and leaders, contribute to care team meetings with other disciplines inclusive of nursing, ST, PT, OT, psychology, MD, and social work. I have also been exposed to innovative therapeutic interventions such as the Lokomat, FES bike, kinesiotaping, and dry needling.

This facility is representative of my fourth and final clinical affiliation and is an acute care facility within Novant Health. The demographics of my patient population were inclusive of oncology, CVA, SCI, TBI, COPD, acute respiratory failure, debility, and post-op orthopedic. I was given the opportunity to observe two surgeries, inclusive of a carotid endarterectomy as well as a double coronary artery bypass surgery with a bioprosthetic mitral valve replacement. I was trained in the EPIC database for the purpose of electronic documentation of patient progress in regards to therapy. I became adept in analyzing patient charts effectively and efficiently in reference to potential changes in status and/or precautions/contraindications to physical therapy interventions.

This facility is representative of my second clinical affiliation and is a skilled nursing facility (SNF). I was introduced to and able to become efficient with electronic documentation. I was also given the opportunity to participate in care team meetings to discuss patient progress and plan of care with fellow clinicians, inclusive of OT/OTA's, ST, and PT/PTA's.

This facility is representative of my first clinical rotation and is an outpatient setting. The primary demographics of my patient population included diagnoses of THA, TKA, TSA, ACL reconstruction, and SLAP repair. I was given the opportunity to implement individualized treatment interventions in compliance with the plan of care, carry a heavy patient load while managing several patients per day, and lead therapeutic interventions utilizing aquatic therapy.

As a rehabilitative technician I assist therapists in therapy sessions. Types of assistance include, but are not limited to, fitting and constructing diagnosis-appropriate wheelchairs, assisting a patient with ambulation, participating in therapeutic activity with a patient, taking care of patient needs, documenting therapeutic charted minutes, calculating therapists productivity, and overseeing and assisting patients with nourishment.

As an operations ambassador, I monitor the facility and maintain a safe environment. This is accomplished by entering each patron into the system and granting access only to those eligible. In addition, I am CPR/AED certified so that in case of an emergency, I can serve as a first responder. I am also responsible for lending equipment.

As an office assistant I create documents via Microsoft Word, Excel, and PowerPoint. I also make various copies of documents and sort/organize transcripts. On occasion, I run errands between departmental offices delivering paperwork, sending and receiving messages, and delivering important documents such as private mail.
