LOCAH is a nonprofit, 501 (c)(3) community based organization volunteer driven in Columbus, OH. Our organization is committed to excellence in care coordination during care transitions, primarily those related to current or recent hospitalizations. Our vision to educate, advocate, and elevate communication for Columbus area professionals to best match the needs of those being served. We promote collaboration between hospitals and post acute service providers. Together we identify pressure points & opportunities in the care transition process. With identified issues, we seek solutions to reduce or eliminate pressure points all-together. We strive to provide resources & tools that allow professionals to work more effectively, simplifying communication & coordination of care.

Recovery Palooza is an annual addiction and mental health event that takes place in Columbus Ohio. We are a community collaboration, committed to access of treatment for an underserved population.
