Pharmacy Clinical Specialist, Copd Care Transitions
CurrentTargeted care to the COPD population is a focus due to penalties faced through HRRP, an initiative of Medicare to reduce readmission rates in several key patient groups. I helped improve the care process for our interdisciplinary team of pulmonary physicians, advanced providers, and respiratory therapists, which not only coordinates inpatient services and care transitions, but also is working to reform the best practices related to COPD patient care system-wide. I wrote and implemented collaborative practice agreements with our pulmonary physicians, streamlining patient care and demonstrating a cost savings of half a million dollars directly related to our interventions. I helped develop a community based pharmacy residency program for our health system, serving as a preceptor as we worked through our initial accreditation, ultimately becoming the residency program director. As the first clinical specialist from the ambulatory pharmacy team, I bring ambulatory knowledge and perspective to predominantly acute care teams, bridging the gap between departments.
