- Complete, in a timely manner, all required assessments, treatment plans. and additional clinical documentation for each individual served thoroughly, with use of trauma informed practice. Ensure that assessments are thorough and inclusive of family and medical history.- Based on results of assessment, develop a person and family-centered treatment plan (lAP) in collaboration with the individual served, parent/guardian/caregiver(s), and with required consent, in consultation with other providers.- Provide individualized treatment that may include working with the individual served, entire family, or a subset of the family, to implement focused, evidence-based interventions to advance therapeutic goals or improve ineffective patterns of interaction- Provide Individual and Group Therapies as assigned, as applicable based on site and program requirements, and as assigned by supervisor.- Facilitate treatment planning discussions to include input of all members of Treatment Team, family members, agency workers, and the individual. Monitor progress of individual served in meeting goals and objectives. Additionally support that treatment through facilitation with other team members of access to community resources.- At family's request and if program supported for site specific role, accompany the family to meetings about the individual's behavioral health treatment needs in schools, day care. foster homes, and/or other community-based locations.- Participate in Discharge process, including providing support for the individual served and family, work with agencies to identify most appropriate aftercare services (if applicable); work with aftercare provider to facilitate transition
