- Clinical pharmacist on the inpatient stroke unit and ICU- Responsible for reviewing medication orders, resolving drug therapy problems, promotingantimicrobial stewardship, provision of drug information and support of efficient drug distribution- Promote safe medication practices on each unit and follow-up on medication incidents- Facilitate discharge medication reconciliation, educate patients on medication changes and communicate changes to the community pharmacy and family physicians - Educate and collaborate with pharmacy and medical students- Experienced using Cerner Millennium EHR platform

- Experienced working in the ICU, General Medicine and Surgical floors- Responsible for reviewing medication orders, resolving and preventing drug related problems,promoting antimicrobial stewardship, medication reconciliation, patient-centered continuity of care,and provision of drug information- Attend and contribute to daily interdisciplinary bullet rounds, as well as daily antimicrobialstewardship rounds with the infectious disease physician while working in the ICU- Providing direct patient care educating patients on oral chemotherapy regimens and adjunctmedications in the outpatient hospital pharmacy- Responsible for supporting drug distribution throughout the hospital including central evening,weekend and on-call shifts- Experienced using Meditech EHR platform
