Willow Park Assisted Living is the ONLY assisted living in Idaho with a certified Joint Commission Clinic. What does that mean? - Now our residents can visit a Nurse Practitioner (with hospital resources available) at our community and their Primary Care Physician is informed and updated (proactive healthcare).- Utilization of ER services for acute illness, diminished. - Bounce-backs to Hospitals eliminated.- ER dumping non existent at Willow Park.Developed the "transition coordinator." Coordinator works with our families to navigate the healthcare system, before and after discharge. The coordinator works with the hospital case manger for a seamless discharge. The coordinator prepares and guide our families to the best rehab option for that family. Helping them understand the process and requirements and restrictions. By partnering with our families we ensure they are receiving the best care options available, during and after hospital stays. Our residents and families should never be alone to navigate the healthcare labyrinth. Occupancy: Low 70s to 100% or always within top two communities within our company. Designed the Logo for Blue Harbor :) Developed our Memory Books (a collection of memories & pictures of residents at our community who have past- provided to families to let them know how much we loved and cared for their loved ones.)
