Responsible for the ongoing success of quality improvement efforts to include meeting the Meaningful Use measures and Patient Centered Medical Home standards. Collect and analyze available data to improve outcomes and comply with reporting requirements. Provide direct supervision to patient support position such as referrals, outreach, and enrollment. Help create and maintain an environment of productivity and respect.

Facilitate the creation of highly functioning clinical practices (50+ across western Colorado) to focus on maximizing the integration and effective utilization of Health Information Technology that improves the cost, quality and experience of care.
