The Team Lead works in collaboration with case managers and other team members to ensure a high-quality and appropriate level of care. While adhering to specific procedures and policies, the Team Lead will assist to empower the adults with serious and persistent mental illness by directing services toward achieving the goals necessary for success and satisfaction in the community. The Team Lead will also support the Regional leaders in implementing systems for Corporate and Divisional goals. - Work closely with Case Managers to ensure that they are responsive to consumer needs and effectively provide the services that they most benefit from.- Support Case Managers in daily problem solving regarding challenges that arise on a daily basis by working directly with consumers in crisis.- Review rehabilitation plans and Case Manager interventions at a regular interval, to assess for progress and to recommend adjustments to interventions provided or adjustments to the rehabilitation plan as appropriate.- Utilize a psychiatric rehabilitation approach, a demonstrated understanding of Community Support Services and the recovery model in direct work with consumers and in guiding and training Case Managers.- Provide crisis intervention services in the homes of consumers as needed.- Ensure that services provided are consistent with NJ DMHAS CSS regulations.
