Senior Practice Transformation Manager
CurrentThe Senior Practice Transformation Manager is the primary interface between Federally Qualified Health Centers (FQHC) and the ACO. The role encompasses relationship management, technical/knowledge enablement/assistance, and transformation to a value-based model of care. The Senior PTX Manager understands all of the components of the ACO, including quality, risk adjustment, population health management, care management, and community health. The Senior PTX Manager will escalate and connect health center contacts with the appropriate subject matter expertise as indicated.Key Responsibilities:* Manage a portfolio of 19 FQHCs in ACO REACH and MSSP contracts* Training/onboarding of FQHC staff in the ACO model* Training and onboarding of new PTX Managers* Regular touch points with all levels of staff at the FQHC, C-suite to front line* Messaging and facilitation of each FQHC's transition to value-based care and primary care capitation* Troubleshooting of population health platform issues* Performance management with respect to risk adjustment, quality, annual wellness visits, and other programmatic areas of the ACOAccomplishments:* Partnered with FQHCs to meet requirements of the new MassHealth waiver (go-live 4/2023); all member health centers meeting requirements in advance of go-live.* Participate in creation and delivery of pertinent talking points relevant to FQHC success in the ACO.* Successfully onboarded 2 new FQHCs into MassHealth ACO, facilitating meetings and ensuring availability of subject matter expertise.* Onboarding and implementation of 9 out-of-state FQHCs joining C3's Medicare MSSP and ACO REACH contracts (go-live 1/1/2025)* Onboarding and implementation of 17 out-of-state FQHCs joining C3's Medicare MSSP contract (go-live 1/1/2024)* Onboarding and implementation of 1 new out-of-state FQHC joining Medicare REACH contract (go-live 1/1/2024)* Ongoing performance management of all FQHCs in Medicare contracts (ACO REACH and MSSP)
