The Senior Practice Transformation Manager is the primary interface between Federally Qualified Health Centers (FQHC) and the ACO. The role encompasses relationship management, technical/knowledge enablement/assistance, and transformation to a value-based model of care. The Senior PTX Manager understands all of the components of the ACO, including quality, risk adjustment, population health management, care management, and community health. The Senior PTX Manager will escalate and connect health center contacts with the appropriate subject matter expertise as indicated.Key Responsibilities:* Manage a portfolio of 19 FQHCs in ACO REACH and MSSP contracts* Training/onboarding of FQHC staff in the ACO model* Training and onboarding of new PTX Managers* Regular touch points with all levels of staff at the FQHC, C-suite to front line* Messaging and facilitation of each FQHC's transition to value-based care and primary care capitation* Troubleshooting of population health platform issues* Performance management with respect to risk adjustment, quality, annual wellness visits, and other programmatic areas of the ACOAccomplishments:* Partnered with FQHCs to meet requirements of the new MassHealth waiver (go-live 4/2023); all member health centers meeting requirements in advance of go-live.* Participate in creation and delivery of pertinent talking points relevant to FQHC success in the ACO.* Successfully onboarded 2 new FQHCs into MassHealth ACO, facilitating meetings and ensuring availability of subject matter expertise.* Onboarding and implementation of 9 out-of-state FQHCs joining C3's Medicare MSSP and ACO REACH contracts (go-live 1/1/2025)* Onboarding and implementation of 17 out-of-state FQHCs joining C3's Medicare MSSP contract (go-live 1/1/2024)* Onboarding and implementation of 1 new out-of-state FQHC joining Medicare REACH contract (go-live 1/1/2024)* Ongoing performance management of all FQHCs in Medicare contracts (ACO REACH and MSSP)

The Health Center Performance Manager (HCPM) is the primary interface between Federally Qualified Health Centers (FQHC) and the MassHealth (Medicaid) ACO. The role encompasses relationship management, technical/knowledge enablement/assistance, and transformation to a value-based model of care. The HCPM understands all of the components of the ACO, including quality, risk adjustment, population health management, care management, and community health. The HCPM will escalate and connect health center contacts with the appropriate subject matter expertise as indicated.Key Responsibilities:* Manage a portfolio of 5-6 community health centers* Training/onboarding of new FQHC staff in the ACO model* Regular touch points with all levels of staff at the FQHC, C-suite to front line* Messaging and facilitation of each FQHC's transition to value-based care and primary care capitation* Troubleshooting of population health platform issues* Review monthly DSRIP budgets with Finance departmentAccomplishments:* Project manager in initiative to increase provider empanelment at each FQHC. Phase 1 determined successful empanelment practices and phase 2, currently being scoped will be dedicated to implementing changes in empanelment practices.* Scrum Master/project manager for internal Scrum team, facilitating internal IT-related projects* Obtained Asana Ambassador status

Empower Seniors Fall Prevention and Home Safety is dedicated to decreasing the uncertainty of aging in place versus the need to move into long-term care facilities by providing information and resources to older adults and their families. Proactive management will increase the success of aging in place for older adults and keep them in the community.

Provide home care occupational therapy evaluation and treatment to individuals in their homes. Assessment of home environment and individual's ability to function in that environment. Supervision of occupational therapy assistants in plan of care.

Works across departments within the PACE organization to develop and implement change initiatives that drive quality of patient care, decrease utilization, and increase long-term financial viability.Accomplishments: • Project manager as part of team charged with opening of new state-of-the-art PACE center including logistics of transportation and staff/participant reassignments. • Lead project manager in patient experience initiative charged with increasing patient experience and decreasing voluntary disenrollment from the program• Project manager in leadership role implementing the emergency management program for PACE. Prior to this project, the PACE program did not meet regulatory compliance. Successfully worked with PACE Administration and staff to implement emergency management program that met CMS regulations.

Clinical oversight of rehabilitation department within the PACE program. Employee training in new initiatives. Project management within East Boston Neighborhood Health Center's PACE program, focused on quality, process, and customer improvement. Involvement in health and wellness programs for PACE participants.

Employee training and oversight of rehab departments within assisted living settings, including identification of residents who benefit from rehabilitation intervention. Educating rehab staff in new initiatives, including dementia, mild cognitive impairment and Wii rehabilitation. Territory included Massachusetts, Connecticut and Rhode Island.

Management of rehabilitation department within an assisted living setting. Provided skilled occupational therapy treatment to residents. Rehabilitation agency administrator, responsible for annual agency analysis and reporting to regional manager. Education of staff on program roll outs within the rehabilitation agency.

Identification of individuals who would benefit from assistance with planning for financial events, including retirement and education. Presenting financial information and calling individuals via phone to establish sales relationship.

Provide skilled occupational therapy to individuals aged one to 100+ post injury or surgery. Rehabilitation focused primarily on the elbow, wrist and hand.

Provided skilled occupational therapy in an industrial rehabilitation environment. Focus of rehab was to get individuals back to work after injury. Conducted functional capacity evaluations to contribute to determination of remaining on long-term disability versus returning to work.

Provide skilled occupational therapy for adults aged 50 and older in adult day health centers. Rehabilitation provided in multiple locations.
