The Senior PTX Manager will escalate and connect health center contacts with the appropriate subject matter expertise as indicated.Key Responsibilities:* Manage a portfolio of 19 FQHCs in ACO REACH and MSSP contracts* Training/onboarding of FQHC staff in the ACO model* Training and onboarding of new PTX Managers* Regular touch points with all levels of staff at the FQHC, C-suite to front line* Messaging and facilitation of each FQHC's transition to value-based care and primary care capitation* Troubleshooting of population health platform issues* Performance management with respect to risk adjustment, quality, annual wellness visits, and other programmatic areas of the ACOAccomplishments:* Partnered with FQHCs to meet requirements of the new MassHealth waiver (go-live 4/2023); all member health centers meeting requirements in advance of go-live.* Participate in creation and delivery of pertinent talking points relevant to FQHC success in the ACO.* Successfully onboarded 2 new FQHCs into MassHealth ACO, facilitating meetings and ensuring availability of subject matter expertise.* Onboarding and implementation of 9 out-of-state FQHCs joining C3's Medicare MSSP and ACO REACH contracts (go-live 1/1/2025)* Onboarding and implementation of 17 out-of-state FQHCs joining C3's Medicare MSSP contract (go-live 1/1/2024)* Onboarding and implementation of 1 new out-of-state FQHC joining Medicare REACH contract (go-live 1/1/2024)* Ongoing performance management of all FQHCs in Medicare contracts (ACO REACH and MSSP)
