Plan, monitor, enforce, and direct the overall health, living environment, finances, social needs, and services for forty-six disabled adults. Scope of the position includes: enforcement of court order, indirect supervision of facility staff and clients, goal development and evaluation, team building, administrative organization & maintenance.•Oversee ten healthcare facilities in Tarrant County, which include nursing homes, an assisted living facility and HCS and ICF group homes. Approve needed services in order to prevent abuse, neglect and/or exploitation. •Conduct monthly face-to-face visits with each client to monitor finances, person directed plan, medical needs, living environment, social needs and overall status. •Develop and implement initial annual care plan and Person Directed Plan to ensure clients receive appropriate services. Also, ensure annual Medicaid renewal application is completed timely in order for benefits not to be lost. •Respond to emergencies, which include hospitalizations, behavior crisis or death.

Responsible for assessing child support cases to determine needed appropriate actions. Documented efforts to collect support and gather information to locate the absent parent. •Completed worldwide searches on the non-custodial parent in an effort to locate him or her. •Explained the legal process to both the custodial and non-custodial parent and provided local resources. •Communicated with the assigned local office when legal action needed to be sought. Mailed delinquency notices to the non-custodial parent in an effort to collect child support as well as wage withholding orders to employers in an effort to have wages garnished for support.

Responsible for investigating reported abuse, neglect, and/or financial exploitation of elderly or disabled adults. Focused primarily on determining if the reported condition existed and to what extent it adversely affected the person. Arranged for services and/or needed protection for the victims in substantiated cases in order to alleviate and prevent further mistreatment.•Served as representative for the State of Texas in court cases involving validated cases of abuse, neglect and /or financial exploitation. •Conducted assessments of clients in their living environment, developed service plans, arranged and provided for services in the home in order to alleviate or prevent further mistreatment. •Prepared court affidavits and filed petitions to the Tarrant County Probate Court and Department of Aging and Disability Guardianship program on validated cases of abuse, neglect and /or exploitation in order to alleviate and protect persons from further mistreatment. •Responsible for seeking prosecution against perpetrators on validated cases of abuse, neglect and/or financial exploitation. •Established and maintained relationship with local law enforcement, medical personnel, court personnel and representatives from other various agencies and organizations.
