Support McEwan's Positive Service Coordination (PSC), Addictions Supportive Housing (ASH) and Mental Health case management teams in the delivery of mental health, substance use, housing stabilization, and life skills supports for individuals living with HIV, and trans and non-binary individuals experiencing homelessness and mental health and substance use challenges.

Managed the operation of high support housing and respite programs in support of clients with current and historic mental health hospitalization, significant barriers related to substance use, and physical health, developmental and psychogeriatric challenges requiring 24-hour residential, personal support, and crisis services.

Provided legal case management and advocacy in criminal and family court systems, access to RGI housing through SPP applications and appeals, private market housing supported by TTHAP and COHB, and navigation of health, legal and social assistance systems for individuals and families experiencing gender-based violence and homelessness.

Provided housing stabilization, brief counselling, conflict mediation, crisis support, service referrals, and community development for 96 tenants living with diverse experiences of homelessness, complex substance use, incarceration, mental health and concurrent disorders, dual diagnosis, and risks of eviction.

In partnership with PARC, supported all west-end referrals from the City of Toronto's Coordinated Access program by providing intensive care management, housing stabilization, eviction prevention, and safety planning to individuals recently housed in Rexdale, Black Creek, South Etobicoke and Parkdale. Provided coverage at Sistering's 24-hr drop-in as needed.

Supported individuals experiencing homelessness and risk of eviction through long-term case management, housing access and stabilization, service navigation, and advocacy. Facilitated landlord mediations and represented clients at the Landlord and Tenant Board. Offered weekly drop-ins to provide to assess eligibility and offer short-term case management and crisis support.

Supported a low barrier Extreme Cold Weather Alert (ECWA) 24-hr drop-in in the provision of shelter services, mental health support, crisis de-escalation, overdose prevention and response, and access to overflow respite sites.

Organized the large-scale rehousing of long-term hotel occupants facing eviction due to redevelopment. Secured private market rental units through advocacy and relationship development with landlords, accessed portable subsidies, appealed cancelled RGI applications, and implemented follow-up case management support.
