ChekMeds, Inc. identifies and communicates medication issues for high risk patient populations post discharge in order to prevent unplanned readmissions. While many factors contribute to readmissions, a substantial number are tied directly to post-discharge medication management/adherence. Most hospitals have launched a major initiative to address readmissions. However, few have a way to incorporate the patient discharge data into an automated system that manages the workflow from patient discharge to case/call managers, to pharmacy, to primary care physicians, and intervention management teams. ChekMeds web-based tool provides a secure system that permits each department/team to access the necessary discharge data and all additional calls and conversations post discharge with the patient. Instead of case managers calling and asking very general questions, they can now ask specific questions that are tied directly to diagnosis at discharge. The system has an element of pharmaceutical intelligence that will alert specific medications tied to specific diagnosis. I welcome the opportunity to discuss further.
