- Assessed co-occurring mental health issues and linked clients to relevant area mental health and primary care agencies who assist disenfranchised populations in the Tenderloin/SoMa districts.- Practiced community based case management with marginally housed, severely mentally ill adults who are high users of psychiatric services and require customized treatment.- Provided comprehensive mental health services including psychotherapy, crisis intervention, and outreach while maintaining positive professional relationships with clients.- Worked on a multidisciplinary team for client coordination and continuity of care, utilizing case management solutions including AVATAR as well as office productivity software.
