Strengthened relationships between disability groups, state sporting associations, clubs, government departments, families and individuals.Met with individuals with a disability, including children and their families, to discuss sport and recreation activities in the local community and created pathways for inclusion to their local sports club.Worked in partnership with Access and Inclusion Officer’s in the local council areas of Armadale, Kelmscott, Gosnells, Fremantle, Kwinana and Cockburn to provide a comprehensive resource of inclusive practices to promote mainstream sport and recreation.AchievementsDeveloped ‘It’s a Goal’ Project in conjunction with DSC’s (Disability Services Commission) Local Area Coordination Network in Kelmscott and Southlake to assist families working with LAC’s (Local Area Coordinator’s) to be introduced to their local sports.

The role was to provide leadership for the Southern Region Respite Network in coordinating existing support and information systems, identifying service gaps and undertaking service system enhancement strategies to better meet the needs of people with a disability and their carers.Coordinated specific respite projects that included four Respite and Recreation Expos for the public and analysed and identified service gaps in respite recreation activities reporting to Respite Recreation Working Party. Developed the Sub Regional Recreation Respite Forum exploring potential for partnerships between disability services and sport and recreation providers.Organised, facilitated and chaired a bi–monthly meeting of the Regional Disability Respite Reference Network that consists of fifty represented services.Managed and promoted the Southern Region Respite Website www.respitesouth.org.au, which includes upkeep of data, visual and graphic alterations, navigation upgrades for usability and liaison with host and info exchange.Produced and facilitated internal training materials to CCRC CareLine workers on the Disability Gateway in the Southern Region and provided external training to service providers, carers and general community about disability resources in the Southern Region and use of the website.AchievementsAssisted in development of a reporting system for recording disability data to contribute to the analysis of gaps in services for the Southern Metro Region. The Elder Abuse Prevention Unit provides a state wide service to older people, family members, service providers and anyone in the community that suspects or is experiencing elder abuse. The service operates a 1300 Helpline during business hours and also provide training and education in elder abuse all over QLD. Role of Senior Project Officer was to manage the day to day operations of the program and all staff.Managed day to day operations of the program including four staff. Provided direct training and education to older people and service providers in rural and remote areas of Qld.Maintained and facilitated teleconferencing workshops for the Peer Support Network. Maintained data and used findings to contribute to analysis of elder abuse prevalence in Australia.Assisted older adults through elder abuse hotline to know their rights and created pathways to access mainstream community legal and advocacy groups.AchievementsQueensland representative for ANPEA (Australian Network for the Prevention of Elder Abuse).Participated in the ‘Inquiry into Older People and the Law’, interpreted for Hansard, through the House of Representatives, House Standing Committee on Legal and Constitutional Affairs. Camperdown Community Options Program provided case management as part of the SSWAHS (Sydney South West Area Health Service) funded by NSW Health. The Service provided two programs. The COMPACKS Program for short-term discharge planning and the Community Options Program for long -term advocacy for individuals and their families.Case managed patients in consultation with the multi disciplinary rehabilitation hospital team, starting before discharge and continuing for a short time after discharge, with the aim of facilitating access to mainstream services and setting up rehabilitation care plans for long term recovery.Provided locum social work to the Emergency Department upon request.Case managed clients living in the community with a range of behaviours and illness.Achievements Provided ongoing information about Elder Abuse in NSW to the Office of Seniors Interests NSW.Implemented first elder abuse education of its kind in NSW to older adults in the Marrickville and Burwood local government areas.Advised SSWAHS (Sydney South West Area Health Service) health policy on elder abuse.Facilitated training workshops on elder abuse for social workers at RPA hospital.Provided regular training in elder abuse to Aged Care Assessment Team (ACAT).

Lane Cove Community Aid Service provides service delivery to Home and Community Care(HACC) recipient's - older adults, people with disabilities and their carers.Assessment, case coordination, referrals, informal counselling and advocacy for individuals and families.Co-ordinated shopping service and linen service for assessed individuals, including liaising with volunteers.Maintained database, statistics, prepared reports, case notes and provided monthly and annual reports to the board.AchievementsCo-ordinated the research into legislation in relation to Elder Abuse in NSW in conjunction with COTA (Council on the Ageing).
