The Healthy Age Friendly Homes Programme is a National Programme, funded by the Department of Health, located in each Local Authority and managed by Age Friendly Ireland ( a shared service of Local Government, hosted by Meath County Council).As Local Coordinator, I carry out home-based assessment with older people living in Dún Laoghaire-Rathdown, to asses their needs for the future, assist with grant applications, rightsizing, assisted technology, working with HSE and other agencies to improve services aimed at >65years. Working in partnership with older person , I identify and design a range of supports to enable the older person to continue living independently for as long as possible. This may involve signposting to services/information, support with grant applications for housing adaptations or energy assessments, facilitating introductions to other appropriate community and volunteer services, tracking progress on agreed actions, to enable and empower older people to live in their own home with dignity and independence, for as long as possible.Referrals for this service can be submitted through the National Office, by phone, post or email.
