Connie Cheren, RN, MSW and President/Founder of Partners for Care, is a career nurse and social worker (Master of Social Work) with a passion to serve. Connie has been a Christian since her teen years, but recently felt God leading her into a mission field which would challenge her faith, yet draw upon her 35 years’ experience as a nurse, social worker, administrator, and organizer. Partners for Care exists due to this blend of her skills and passions, intersecting with God's calling.Connie's professional accomplishments include being the winner of the prestigious Robert Wood Johnson /Harvard University Award for Innovation in Local & State Government for her work in the field of long-term care for the elderly. This program, well- known as the 6-Star Program, won national recognition from other states and also won the Harvard Innovation in Local and State Government Award. Also, redesigned the nursing component of Medicaid reimbursement to pay the provider for implementing effective restorative programs in contrast to other case-mix systems that increase the level of reimbursement as the resident gets "sicker."
