Responsible for all public awareness efforts of the program and includes marketing of services to providers, referral source and the community at large.Community Education/Intake Coordinator· Supervises the evaluation of potential enrollees to the program to determine participant needs and eligibility for enrollment. · Supervises the scheduling of assessments that are completed in the home and at the PACE Center. · Supervises the PACE Medicaid application process and serves as a liaison between the local Departments of Social Services to assure timely completion of PACE Medicaid applications. · May participate in the Interdisciplinary Team, Management Team and other related meetings and activities. · May assist with coordinating the PACE Center’s Participant Advisory Council quarterly meetings. · Supports the development, implementation and evaluation of the overall marketing strategy for the recruitment and retention of PACE participants for the building and maintenance of an optimal enrollee census. · Recommends potential public relations problems and advises his immediate manager and if applicable program leadership of appropriate preventative actions. · Supports the maintenance of administration and relevant reporting and planning systems.
