Support Coordinator
Assisted individuals with Developmental Disabilities in gaining access to needed programs and state plan services, as well as medical, social, educations, and other services needed. Provide compassionate support to the client and their family. Conduct yearly individual service plan meetings with providers, supports, and families to ensure client goals are met and client services are provided, Supportive counseling/consultation is provided to client and family. Monitoring conducted with a monthly phone call, quarterly face-to-face visits, and annual home visits to complete state reporting. Effectively use technology in communication with consumers and providers with preparing service plans, budgets, and other paperwork necessary. Work with a variety of people and possess multicultural sernsitivity.
