Established a Clinically Integrated Network within a 523 bed multi-hospital system on the Treasure Coast from the ground up with the Executive Director. We implemented a clinical strategy, business plan, and operating policies which have created substantial revenue and care based outcomes. Focusing on direct patient care programs we have changed the outcomes of our most complex patients. Year one of our MSSP Track 1 ACO:• Acquired Attribution of 20,000 beneficiaries from Employed and non-employed practices• Minimized readmissions to less than 9 percent consistently ( compared to 13 percent system wide )• Created transitional care and chronic care management programs producing downstream net revenue of $2.4 for network physicians and reduced hospital readmission penalty by 42% and so much more
