I am the team lead for illinois red cross Chaplains. I am a volunteer crisis counselor with crisis textline. I work with both home care and hospice care. I meet with patients and family to discuss there options when they are eligible for either home care or Hospice care. I am an advocate for them. I provide education about what is next and the process. I start the process of home care or Hospice if eligible and everyone agrees. I then check up on the patient and make sure they have everything. I also provide companionship as well. I grow relationships with the patient and family, that way we can do what is best for the patient. Like I said, we are the advocates for them and try very hard to get what they need to help the patient in anyway I can.
