Responsible for performing daily tasks such as checking prescriptions, counselling on prescription and OTC medications, conducting medication reviews, constructing notes to physicians in regards to drug therapy problems. Therapeutically check and provide medications to regional hospices: Lisaard and Innisfree House. Involved in an INR clinic monitoring patients on warfarin and making dose adjustments as necessary.

Manage a point-of-care INR clinic; patients work ups for memory clinic involving a practitioner and occupational therapist; respond to physician/nurse practitioner referrals which vary from drug information questions, patient consultations, chart reviews, medication reconciliations; collaborate with practitioners to ensure patients are receiving the best possible care. Preceptor to University of Waterloo PharmD candidates.

Work collaboratively with other health care professionals to provide patients the best possible care. Assist physicians and nurse practitioners with drug information questions, patient consultations, chart reviews, medication reconciliations, and patient follow ups.

Conduct in home medication reviews for complex patients based on physician referrals. Also currently involved in a pilot study with Cambridge Memorial Hospital assessing the impact of pharmacist in home medication reviews post discharge.
