Involves three partners: the public school system, the hospital, and faith-based community.> For each initiative: Reported to and worked with a group of key community members equivalent to a board of directors; coordinated collaboratives and workgroups; collaborated across community sectors to achieve goals; hired and supervised staff and consultants; monitored each initiative's spending and income to stay within annual budgets; wrote grants and reports; developed and maintained relationships with funders; and oversaw evaluation efforts.> For Opportunity Knocks: Managed post-intervention evaluations of two problem areas (obesity and oral health); analyzed this data in context of baseline data and statewide/national data to determine degree of effect of initiative's efforts. Organized cross-sector trainings in evidence-based/best practices to ensure consistent messaging to families. Trainings included Motivational Interviewing for obesity prevention/treatment, assessment and treatment of behavioral health issues of young children, and addressing inappropriate infant feeding behaviors.> Recognized with Team Spirit Award (March 2012) by Middlesex Hospital Family Advocacy Program Division for securing matching funds for Child FIRST program and leading the hospital's partnership with Community Health Resources in the successful application for CT DCF Community Support for Families funding.
