Responsible for the care of an average of 8-9 patients during my shift in the Sub-Acute unit. Chart accordingly for Medicare patients. Manage PCC (Point Click Care) software system, wound care, admission intake, tracheostomy care, Gtube care, medication administration (PO, SUBQ, IM, GTUBE), discharge patient/family teaching, provide therapeutic communication for patients and family members, team member of rapid response team, provide post mortem care, palliative care. I have also assisted the DSD with the surveillance data collection of antibiotic usage while being the Infection Preventionist.
