• Establishes strong working relationships with foster home and group home providers (agencies), foster parents and CAS workers.• Engages with vouth to help them determine and develop their circle of support.• Assists youth with exploring possible housing options, mentors, and caregivers and determine if positive matches are possible.• Completes a full investigation into possible support networks and people available to the youth, expand circle of support by connecting youth to culturally appropriate groups and engage the extended family or community.• Supports potential caregivers in exploring the possibility of them becoming a caregiver for the youth, navigating concerns, and conduct necessary training with a unique focus on how to support older youth in a home environment.• Completes housing assessments and interviews for safety, evaluation, and appropriateness of the trained and screened potential caregiver. Ensures that safety and well-being practices are implemented.• Provides support to both caregivers and vouth to ensure a successful placement.• Provides all services within an understanding of anti-oppression, anti-Black and Indigenous racism, diversity, LGBTQ+ positive and anti-poverty values and principles.

• Providing specialized day-to-day care and counselling to women and girls who have been impacted by sexual exploitation and/or human trafficking• Providing direct service, assessment and case management services to all youth living in the transitional house using a trauma informed approach • Assesses and works with clients in order to implement a strategic plan of care while providing ongoing support • Facilitates daily life-skills programming • Oversees the operations of the transitional house including monitoring its health and safety, fostering its sanctuary, enforcing house rules, policies and procedures, responding to crisis situations, and other duties as required • Maintains positive relationships and interacts with various segments of the community (CAS, client family members, neighbours, property management personnel, etc)

• Case management of youth between the ages of 18-24 living within a transitional housing setting at Brennan House A.C.T.S.• Connecting youth to appropriate community resources• Attending and supporting youth with appointments where necessary• Facilitating resident meetings and life-skills programming• Conduct unit inspections and weekly check-ins with the youth

• Working alongside other transitional youth workers to provide support to youth ages 16-24 within the Halton Region who are experiencing homelessness or at risk of being homeless• Provide case management to a caseload of youth • Support youth with goals towards obtaining and sustaining affordable housing• Facilitate access to services within the Halton Region such as housing, financial, employment, education and counselling• Meet and attend appointments with youth• Meet with community partners • Work with family members of youth• Attend team meetings as required • Assist with client surveys in regard to feedback about the program
