At the onset of the Transitional Youth Program, Community Living Guelph Wellington hired me to develop and implement a new and specialized program for individuals with dual diagnosis/concurrent disorders. Most of the individuals in the intake process have encountered complex trauma in their personal and family histories; therefore, they were challenging to engage, and trauma-informed care was imperative. We developed trackable, individual client-centred care plans (with a harm reduction focus) with the individual transitioning from the Family and Children’s Services system into a modified, semi-independent support program. This program supports individuals while learning to maximize their strengths and interests in their own home and community while balancing this focus with the awareness that the individuals have been assessed with mental health and/or addiction disorders and provides support for this based on the recovery model. The program emphasizes building hope, resilience, connection, self-awareness, and empowerment in a non-judgmental environment.
