Rotational Band 5 Occupational Therapist: Mental Health Services For Older People (Mhsop)
This role was focussed in a day hospital setting (Assessment and Recovery Unit [ARU]), although I covered the inpatient service when needed. As MHSOP was being reconfigured and the ARU was new, I was involved in the development of the OT service within MDT. Within this role I worked with older adults experiencing organic (i.e. demenitia) and functional (i.e. depression, bipolar disorder) mental health problems, in the ARU and in the communinity. My key activities were:-Supporting clients and families to come to terms with the impact of mental health difficulties and cognitive impairments through engaging with them in psycho-education, within the context of the Occupational Therapy problem solving process and the Recovery focus of the service.-Building staff relations and confidence in the OT service with ARU staff.-Carrying out specific Occupational Therapy assessments, developing client centred treatment plans with clients (and/or their families in the case of severe cognitive impairments) and interventions based on cognitive behavioural, recovery, functional rehabilitation, and psychodynamic approaches. -Writing individual Occupational Formulations based on the MOHO. -Developing a Health and Wellbeing group with nursing staff based on recovery approaches. -Developing a "Leisure Group" for clients who have lost interest and skills in carrying out previously valued activities.-Assisting with superivision of OT student on evaluation placement.-Facilitating "Recovery" in-service training for wider MDT and services.-Contributing to individual case reviews and discharge plans.

Occupational Therapist: Acute Medical
Within this role I covered in acute inpatient wards specialised in medical, surgical, respiratory, and short term physical rehabilitation. Generally, the referrals were elderly clients who had been observed to be having physical, mental, or social difficulties likely to impact upon independent functioning once discharged, for example a person who lived alone with difficulty mobilising after a fall. My responsibilities were:-To respond to all ward referrals, carrying out intial, mobility, transfer and functional assessment where appropriate, including gathering information from family members.-To carry out home environmental assessments and complete reports where necessary.-To make client centred recommendations relating to rehabilitation and equipment needs.-To liase with all members of the MDT regard discharge plans and progress.-To refer on to Social Services OT for complex adaptations or equipment.-To work within legislative and safety guidelines.-To keep accurate paper records on ward and within departmental notes.-Completed Band 5 preceptorship-Engaged with in-service training to maintain CPD standards.
