In this position I provided community based assistance to adults with mental health diagnoses, co-occurring disorders, legal and substance use issues and multiple health concerns in a way that was courteous, responsive, professional, and helpful to the consumer. I assisted consumers with necessary paperwork to obtain or maintain benefits, linked consumers to needed resources, monitored psychiatric stability and medication compliance, assessed risk at each session, coordinated with primary doctors, probation officers and other agencies on the consumer’s behalf and advocated on behalf of consumers or for consumers to obtain necessary services. I performed all duties while maintaining adherence to HIPPA guidelines and using the CHIP system. I was knowledgeable of and able to meet all contractual requirements in a timely manner. I consulted with team members/supervisors as needed to ensure consumer needs were fully met at all times and was able to complete all responsibilities with little to no supervision.

In this position I provided crisis counseling to youth, ages 10-17, and their families on a short term basis. Youth in this program were either at risk, homeless, or at risk of homelessness. I was responsible for counseling shelter residents daily, assessing risk, creating family reunification plans, creating/monitoring short and long term goals, empowering youth and families to work towards goals/reunification, linking to community resources and providing referrals, coordinating to improve overall family communication, advocating for youth/families, attending and speaking at court hearings, coordinating with schools/probation officers, preparing runaway or incorrigibility petitions, answering crisis calls, completing intakes, teaching active listening and other needed skills, educating on diagnoses/medications and offering community presentations to promote the agency. I worked in Genesee, Lapeer, and Shiawassee counties while meeting all deadlines and working with limited to no supervision. I alone was responsible for the operation of the Lapeer and Shiawassee County offices.

In this position I provided in-home services to youth and families involved with juvenile court, Protective Services or Foster Care on a 24/7 basis. Services included counseling youth and families, setting attainable goals with youth and families, crisis intervention, mentoring, attending and speaking at court hearings, generating reports to ensure contractual compliance, identifying and teaching needed skills, leading group sessions to improve skills necessary in the community, linking to necessary personal items, completing necessary paperwork/referrals and assisting youth in identifying appropriate behavior to reduce/eliminate court involvement. This position offered the opportunity to work one on one with troubled youth, help them identify and achieve goals to eliminate legal involvement while working with extremely limited supervision and meeting all contractual obligations.
