Responsible for developing and integrating assessment/clinical care forms into Electronic Medical Record and other systems, as well creating tracking and data reports from the systems. Information Analyst will work to further the development of outcome data reporting used by the operational team. Work to create efficient processes through LEAN Management.

Assist with the leadership of all CBRF & SNF operations. Designated as the Administrator of the CBRF, assuring delivery of quality programs and services to the residents in a safe and secure environment, maintaining positive relationships with residents, family members, staff, volunteers, churches and the local community, and for providing continuity of care and the consistent application of policies and procedures.
