Outreach Directors (ODs) at Oak Street Health, are important leaders in the organization. ODs are responsible for closely developing and managing a team of up to 5 Outreach Associates, one Community Coordinator, and one Community Relations Manager. ODs manage a single clinic location and are responsible for Oak Street Health’s patient growth at that clinic. ODs are expected to review metrics daily, support growth initiatives, and generally ensure the Outreach team operates smoothly and successfully.Responsibilities:-Supervising, coaching and mentoring a multi-disciplinary team-Relentlessly pursuing that team's monthly growth targets through strong leadership, analysis, problem-solving, teambuilding, and coaching-Leading the execution of outreach and marketing initiatives-Advocating for the clinic, team, and patients-Being an ambassador in the local community-Building relationships with other healthcare providers, social/ community organizations, and marketing partners

● Create and maintain partnerships with organizations both within and outside the clinic zone ● Establish connections and build relationships with community leaders that have access to our patient population of older adults on Medicare ● Contact community leaders to create events and opportunities for our Outreach Associates to present OSH and to bring eligible prospective patients into our centers ● Heavy prospecting to service coordinators at senior buildings, police districts, libraries, churches, park districts, senior groups, social services organizations, food pantries, and other community resource centers serving seniors ● Coordinate all details of the events with Oak Street employees to ensure that catering, materials, schedules are all in sync. ● Campaign and event planning to promote OSH and attract potential patients ● Market Oak Street Health brand within all areas of the clinic catchment zone ● Work as part of the outreach team with the goal to gain new leads, prospects, contacts and ultimately patients. ● Create newsletters sent out to clinic partners, collaborators, community organizations as well as clinic based staff and regional managers. ● Create flyers and media materials for events ● Generate weekly events schedule for Outreach Associates, Outreach Liaison and Community Coordinator

● Maintain relationships and partnerships of outside organizations ● Manage staff members (Lunch/recess. after school, summer programming) ● Provide leadership for implementing the Community School Model ● Designs and coordinates student and family support services ● Oversee the after school operations (After school programming) ● Develops, maintains and publicizes a schedule of resources, programs and activities ● Recruit students for programs ● Recruit and develop highly effective program partners ● Maintain accurate records, performance metrics, budgets and data ● Provide programmatic logistical support and new funding possibilities ● Provide assistance to principal and school administration as needed ● Aid with school events and parent communication ● Spanish speaking liaison
