• Supported a project with a large telecom company, $ 9.0 bil in revenue, in its build out of a telehealth platform to serve both payers and other post-acute care service companies. The goal, to help members overcome the digital divide and provide easy, comprehensive digital health. The work resulted in a tablet-based technology with use cases starting with post discharge and health risk assessment. Original pilot was successful and quickly expanded to more patients. • Partnered with an international logistics company to source tablets and other technologies for the use in virtual and remote patient care.• Evaluated information regarding innovative Primary Care models in order to understand how they operate in support of enhanced post-acute care in conjunction with value based payment models in support of two clients. • Supported, a behavioral health company, in their expansion to multiple states. Helped develop the direct sales campaign in alignment with the digital marketing group.• Setup CRM tool for Cavo Health (risk assessment & coding) and helped select the correct tracking metrics for customer engagement and management.• Researched COPD/lung health connected devices for Aire Health’s competitive assessment.
