Senior Case Manager

Promoted to Senior Case Manager after exceeding company expectations in Case Management, systems navigation, and training. Systems included B.U.S, CCM, Case Worthy, BRIDGE, and Peak Pro.B. Coordinated in person and virtual IDT meetings as required by state and federal regulations. Meetings included IDD individuals and up to 15 support team members.C. Managed IDD individual’s service plans by adding, removing, or modifying existing services. To ensure health and safety of the IDD individual, this included the implementation of Right Modifications, the review of existing Right’s Modifications, and the completion of Informed Consent Forms as required by state and federal law. D. Researched, analyzed, and resolved all billing discrepancies in a timely manner to ensure the continuation of all services uninterrupted. E. Maintained accurate and professional communication. This included connecting IDD individual’s with service providers, addressing inquiries, providing updates, following up, and sending required documentation while adhering to all HIPAA laws.F. Executed constant and consistent case file review to reflect accurate and up-to-data information.
