I aim to build the internal capacity and sense of efficacy of communities as I believe the best solutions will come from within the community that live it -- whatever 'it' may be. To achieve this goal, I enjoy working with partners at clinical and community based organizations to improve cross organizational and cross sectoral collaboration as a way to strengthen the regional ecosystem of care for people with complex health and social needs.My time spent doing telephonic case management for people with identified social determinant needs that were seeking care in the ERs was foundational as I learned about the resilience and strength of the Camden community. I have, and will continue, to carry this knowledge with me to other roles and I work more with community and clinical partners to improve systems of care for the people that live in our community.

I provide outreach training and facilitate case discussion and inter-health system coordination to support patients. I am also developing many resources to be disseminated to patients and OB practices to improve patient-centered care practices and increase the capacity of OB offices. Our workflow utilizes a data-driven triage process to identify participants. This triage process requires manual chart review of all patients seen in local emergency rooms with evidence of a possible or confirmed pregnancy. Through a budding partnership with Inspira Health Network and CompleteCare FQHC, I worked to replicate this workflow in regions that do not utilize our Health Information Exchange. This new workflow demonstrates the ability to scale this workflow across the state and beyond. I regularly conduct telephonic outreach for this workflow to provide assistance scheduling follow up pregnancy-related appointments and to address barriers to accessing care using our patient-cost budget. We aim to develop a way to sustain this crucial, proactive workflow as we believe strongly in the impact and importance of this work.I have the privilege to co-chair our monthly Camden Care Management Meeting. This meeting brings together community members, community-based organizations and healthcare providers to learn about community resources and engage in cross-sector collaboration. The monthly programming varies but often features a local organization to share their programs, de-identified case presentations to collaboratively problem solve, and community updates.I also serve on the organization's Diversity, Equity and Inclusion committee. I was able to provide short term, telephonic case management to connect patients with social resources to support their needs. Beyond making referrals, I assisted with the application and follow process and worked to increase the participant's skills and sense of self-efficacy to address future needs, should they arise.Major successes I was able to support include the awarding of $23,000 in back-owed unemployment to a client recovering from long term COVID symptoms, family unification and the development of a formal custody agreement and the successful re-housing of several participants in unsafe or unstable housing situations. I supported a pilot program to support the Safer Childbirth Cities work co-occuring within the department. This involved providing education to the AHC team regarding the expansive WIC services available and the increased referral to WIC services. We utilized the MyResourcePal platform to develop a closed loop referral system with the Camden County WIC office to better support patients eligible for WIC services. Shortly after joining the team, we transitioned to a new calling system and process. I was able to support the quality improvement of this new system and grow the functionality and efficacy of the platform. As new team members were onboarded, I provided training for multiple groups of interns that were supporting screening and navigation processes.
