I am the owner of a new mobile outpatient registered dietary, physical therapy, occupational therapy, and speech therapy clinic, Far North Health Services LLC, serving Anchorage and its surrounding areas. It's a clinic on wheels taking place in patient's homes. Our goals are to optimize the patient's independence in a familiar environment and reduce caretaker's burden with appropriate adaptive devices and training. Our main focus is to provide a continuum of care for patient's as they transition from an acute hospital stay or home health therapy services to home alone, home with family, or to an ALF. I am very excited to be offering this new model of health care to Anchorage and its surrounding area. We take Medicaid, Medicare, Cigna, BCBS, and Aetna.

We provide hospital quality (Arjo brand) mechanical lifts and standing and rising aides (Sara stedy) to Alaskan families who need quality equipment to decrease caretaker burden and improve health related quality of life. We also deliver adaptive equipment (walkers, bedside commodes, toilet risers, etc) to patients homes.

We provide hospital grade lifts and standing lift aids to Alaska families who are caring for a loved one with mobility concerns. We hope to improve access to quality adaptive aides to decrease caretaker burden and improve quality of life.

Patient examination for a variety of musculoskeletal conditions and variety of ages from adolescent to geriatrics; gather information to develop hypothetical diagnosis, evaluation of objective measures to establish physiotherapy diagnosis, prognosis, and plan of care; discuss findings with patient and collaborate to establish goals of treatment; determine whether referral to an additional healthcare provider is appropriate; administer and supervise treatment; re-evaluate and adjust plan of care as appropriate; use of evidenced based practice for interventions; identify any precautions or contraindications for treatment, application of manual techniques; application of modalities such as hot packs, ice packs, or electrical stimulation; progression or regression through therapeutic exercises; maintenance of adequate documentation; determine when client will no longer benefit from PT; perform and document final evaluation to establish discharge and follow-up plan; utilization of appropriate outcome measures to monitor improvement; falls risk assessment and recommendation of appropriate assistive device; assistive device training; set goals and outcomes for individual patients consultation by giving professional opinion to identify problems, recommend solutions, and produce specific outcomes in understandable terms to patient; patient/family education; special tests, range of motion and muscle performance, body mechanics advice for work place set up, lifting, sleeping postures; secretarial and housekeeping duties as needed; routine tasks related to the operation of physiotherapy services; communication with physicians; delegate plan of care to support personnel; supervise and direct support staff in designated tasks; adjust patient education techniques according to the patient’s preferred style of learning; set goals and develop plan to achieve goals.

Chart review and patient examination of a variety of musculoskeletal, neurological, integumentary, and cardiopulmonary pathologies in the geriatric population in the acute rehab setting; discuss findings with patient to establish goals of treatment; identify any precautions or contraindications to treatment; lift transfers, positioning in bed; set functional goals and outcomes; discuss with patient and caretaker regarding outcome achievement required for discharge; discuss discharge plan including destination, equipment needs, and home care services; developed plan of care to achieve specified goals; education and body mechanical training in ADLs; home consultation to determine adaptations necessary; ace wrapping residual limbs; guidance in selection and use of assistive device; motor function, cranial and peripheral nerve integrity, reflex integrity, sensory, joint integrity and mobility, pain; Assisting with departmental activities; participation in weekly team meetings with interdisciplinary team to discuss and coordinate services; family training, secretarial and housekeeping duties as needed; routine tasks related to the operation of physiotherapy services; therapeutic activities to enhance functional performance; determine when client will no longer benefit from PT; adjust teaching techniques for rate and style of learning; maintain accurate and concise documentation according to industry standards; bill accurately to capture the charges for services performed; therapeutic activities to optimize functional performance; exceptional time management skills to manage full case load

Chart review and patient examination for a variety of musculoskeletal, neurological, integumentary, psychological, and cardiopulmonary pathologies in the adult and geriatric population in an acute care setting; identify any precautions or contraindications to physiotherapy intervention; set achievable functional goals and outcomes; assessment of vital signs; instruction in energy conservation techniques; guidance in selection, sizing, and use of assistive device; therapeutic activities to enhance cardiopulmonary function and decrease effects of bed rest; positioning in bed to protect integument; determine if patient is appropriate for continued PT services while in/out of the hospital; adjust patient education techniques according to patient preferences; assessment of ergonomics and body mechanics, assessment of gait, locomotion, and balance; appropriate communication with support personnel; promote patient’s understanding of current condition, impairments, functional limitations, and disabilities; recommend the appropriate discharge location; identify assistive equipment necessary for smooth transition home; varied method of teaching to suit individual learner’s unique characteristics; informed, educated, and trained PT clients and families in order to promote and optimize PT services; identify elements necessary for a smooth transition to home, safety awareness; maintain adequate documentation according to industry standards; code accurately to capture the charges for services performed; therapeutic activities to enhance functional performance; exceptional time management skills; instruct nursing in ROM, bed exercises, transfer techniques, and ambulation assistance level necessary
