-Create and maintain deep engagement with, and support for, patients and their family members during their cancer journey -Conduct in-person visits with physicians, nurses, social workers and other providers at cancer centers and community oncology offices, as well as other community-based organizations to build relationships and introduce them to LLS’ mission and core services to increase patient referrals-Utilize time management skills to create and implement an outreach plan and conduct visits at over 50 hospitals, oncology offices, and community based organizations throughout the year in both Washington and Alaska (WA/AK)-Conduct community-based education programs to diverse audiences including patients and healthcare professionals by creating and aligning project plans, monitoring scope, schedule and budget of projects-Manage day-to-date administration of programs to ensure efficient delivery of programs-Draft reports to assess and evaluate programs to provide to grant organizations for annual reporting-Engage stakeholders in the healthcare community to promote education programs and projects-Continuous budget management of multiple projects and deadlines-Assessment of ongoing needs and gaps in access, services, and education for patients and providers within WA/AK-Present Patient Access data and research initiatives to chapter staff to promote the mission and goals of LLS-Provide information, advice, and consultation regarding health and financial resources toa high volume of inquiries from patients, families and healthcare professionals-Identify opportunities to better reach underserved communities through provider and community outreach-Build and manage mission committees of healthcare professionals and patients to create and implement new strategies to serve communities.

-Responsible for assisting patients and families that visit the center with their requests for health information, resources and educational materials-Work closely with clinical patient navigators, nurses, social workers, chaplains, patient relations and providers as well as community partners.-Responsible for coordinating resources across the center, connecting with systems of care across the center and continuation of the patient experience.-Assess needs of patients and appropriately referring them if needs are out of scope for the resource coordinator role.-Maintain current inventory of materials for the Resource Center, updates information files. Helps to identify resource needs.-Present to patients, families and staff on resources and educational/informational topics.-Assist manager in tracking Resource Center usage and trends. -Practice excellent customer service skills in a confidential and professional manner.-Help resource center visitors with computer searches and computer questions, business center needs and lending library assistance

• Act as a contact for patients and family members, referring physicians, and outside health facilities by providing information regarding the hematology and hematologic malignancies clinic services.• Manage incoming new referrals via incoming calls, electronic referrals and faxes• Manage all hematology providers’, fellows’, and residents’ schedules• Coordinate and schedule appointments for patients; match patients with appropriate providers based on diagnosis• Follow department processes for registration and patient financial clearance, including but not limited to creating referrals in Epic and following the concerning insurance process• Identify any areas of concern/anticipated needs (transportation, support care, social issues, etc.) of patients and triage appropriately• Receive and respond to patient and physician concerns and complaints• Request medical records from referring physicians and outside institutions; review for content and completeness; organize in designated categories for clinician review• Operate multiple electronic medical records and scheduling systems on a daily basis
