Manage the planning, coordinating, and tracking of internal and external referrals for patients within the Oklahoma SBIRT (Screening, Brief Intervention, and Referral to Treatment) Project. Maintain a resource database and assess, monitor, and evaluate community resources related to their ability to meet patient needs. Establish and maintain positive rapport with patients and referral resources to keep track of their progress and to ensure patient satisfaction. Accurately track and document required GPRA (Government Performance and Results Act) data according to SAMHSA guidelines. Perform 6-month and discharge assessments. Coordinate with Behavioral Health Consultants regarding patient care needs. Track patient care and follow-up with referrals. Coordinate with referral agencies to schedule appointments for patients. Help ensure compliance with appointment attendance by following-up with patients prior to scheduled appointments. Assist patients by finding solutions to impediments to them receiving necessary treatment.
