Services include timely medication order verification, clinical pharmacokinetic monitoring of high-risk medications, medication reconciliation and assisting nurses and physicians with medication questions and concerns. I have spent the last 13 years providing pharmaceutical care services on various inpatients units (OR, ER, PACU, ICUs, Medicine-Surgery Units). I pioneered the decentralized pharmacist model role at our hospital in the Post Anesthesia Care unit where I built key relationships between pharmacy and nursing establishing our important role and value of having a decentralized pharmacist. Due to the success of this model, the department fully implemented the decentralized clinical staff pharmacist model.My colleagues view me as knowledgeable, detail oriented, thorough, always focused on safety and efficiency.
