Current• Apply principles of cultural awareness and an understanding of social identities to promote diversity, equity, and social justice in Health and human services.• Case management support for up to six clinicians, each with a caseload of thirty-five clients. • Provide a wide range of comprehensive, integrated care with a trauma-informed approach to adults with serious mental illness, co-occurring substance use, primary care, and other complex issues. • Complete client referral for client linkage to community resources for the continuum of care for specialty mental health resources. • Facilitate family meetings to develop discharge planning in the community pre- post-hospital psychiatric admission. • Coordinate weekly check-ins with clients in support of treatment plan goals.• Trainer for newly hired Social Worker III employees• Evaluate the degree of community functioning and emotional distress of clients. • Ensure time-sensitive documentation is submitted with established guidelines.• Work collaboratively within a multidisciplinary team.• Serve as a client advocate by being the voice for patients when dealing with police, health services, and other organizations. Ensure clients receive the care necessary for their recovery and well-being. • Conduct weekly home visits to facilitate evidence base practice interventions and assist clients with safety planning. • Participate in Crisis Intervention training.• Facilitate weekly Life Skills, and psychoeducational groups, for skill development to improve clients' daily living. • Provide interpretation support from English to Spanish and Spanish to English for clients during psychiatry appointments.
