I’m responsible for over seeing all patients in the hospital who require on-going support on discharge and ensuring this happens in a timely manner.Each day brings up unique challenges that require an independent approach to make sure patients get the best care possible.Using my experience in high pressure environments and key attributes such as: prioritisation, project management, people management and problem solving I achieve the following:- Liaise and manage multiple stake holders on the patients behalf. This involves negotiating with different divisions to get the best care for the patient within the budget and resource of the hospital and other caring parties.- Directly responsible for complex cases across the trust- Support and manage the ward-based team with discharge planning and any issues arising. - Manage staff sickness/return to work, HR issues, and on boarding new starters- Coordinating internal resources and 3rd parties to flawless execution of patient care and discharge- Build relationships and empathise with patients and relatives in what can be stressful periods- Collate, and report on data to aid daily decision making in the trust and for long-term improvements.
