I serve as the Agency's Mishap Investigation Program Manager. Duties include the following:1. Provide advice, support, and oversight for NASA mishap investigations and provide guidance to the Agency for mishap investigation inquiries. Support the formation of mishap investigation boards for all Type A and Type B mishaps. This includes ensuring that the mishap investigation appointment letter conforms to NPR 8621.1, NASA Procedural Requirements for Mishap Reporting, Investigating, and Recordkeeping, verifying that mishap investigation board members have the proper experience and training, ensuring that the required advisors are selected and approved, verifying that the NASA Safety Center is prepared to support the mishap investigation, and ensuring that the mishap investigation board has the tools needed to conduct the investigation. 2. Critique mishap investigation board products. This includes reviewing draft mishap investigation board products (Root Cause Analysis, Mishap Status Reports, Mishap Warning Action Response issuances, mishap investigation report), reviewing the final mishap investigation board report for content and compliance with NPR 8821.1, and preparing an endorsement memorandum for signature of the Chief, Safety and Mission Assurance.3. Manage mishap investigation tools and training. Develop Interim Response Team training, supporting root cause analysis training, and serving as administrator for the NASA Mishap Investigation Website.4. Work with the Agency mishap investigation community to update Agency requirements. Work with the Office of Procurement to update NASA Federal Acquisition Regulation clauses to reflect agency requirements.5. Lead the NASA Mishap Investigation Working Group.6. Review Mishap Preparedness and Contingency Plans.7. Provide briefings and information on mishap reporting, investigating, and recordkeeping within and outside of NASA.
