Report any changes in the client’s condition to healthcare professionals.3. Ensure adherence to care plans and medication.4. Aid clients with mobility issues, including transferring from bed to wheelchair, and support the use of mobility aids like walkers, canes, wheelchairs and commodes.5. Prepare nutritious meals according to dietary needs and preferences. Assist with feeding as necessary and ensure proper hydration.6. Perform light housekeeping duties, including sweeping, mopping, laundry, and organizing. Run errands and assist with grocery shopping.7. Provide emotional support and companionship. Engage clients in activities that promote mental and emotional well-being.8. Accurately document care provided and any changes in the client’s condition in the visit summary. Communicate effectively with clients, families, and healthcare professionals.9. Provide basic first aid and CPR as needed. Recognize and respond to emergencies promptly and effectively.
