Throughout my experience I worked with adults aged 65+ transitioning back home following an acute hospital admission. I gained valuable experience working alongside my Clinical Educators prescribing equipment, formulating home modification recommendations, organising equipment trials, and loading with relevant myAgedCare providers to best support my clients during their transition. I had the opportunity to prescribe long-term equipment using GEAT2go and the Medical Aids Subsidy Scheme, as well as regular trials of equipment through providers such as aidacare and Independent Living Solutions. During my time I gained excellent experience working within a multidisciplinary team, delivering comprehensive case reviews alongside physiotherapists, speech pathologists and social workers and many more. Working within the community afforded me many opportunities to connect with clients in their safe and comfortable space; a method that I found to best support client-therapist connection. With invaluable guidance, I was able to deliver cognitive assessments; MoCA, safety and judgement questions, functional kitchen assessments and observational assessments. Whilst maintaining and independent caseload of 12 clients, I gained experience with time management, prioritisation and negotiation. I communicated with relevant interstate stakeholders in my clients care to best support their safe transition home. A large proportion of my role was succinctly and efficiently documenting any direct and indirect client contact. I completed this in written charts and occasionally via IeMR.
