Provides support to Case Management staff and the Director of the department.Indexes and creates authorizations in the appropriate software program. Works closely with RN Case Managers to ensure accurate and complete required documentation that meets risk management and regulatory requirements. Independent Living Program Caseworker
Case Managed Klamath and Lake County Foster Youth Independent Living Program with a case load of 45 youth in care and 15 adults under 24, who were receiving services, supports and monthly rental stipends.Piloted PeerUp! community based group along with transitional living program for Klamath and Lake county. PeerUp Klamath had 6 fully trained Peer support specialist with previous lived experience. Youths ages 11-18 began to engage and the program had 12-15 youths attend on average each week. Pilot programs were so successful that they have begun statewide rollout. The ILP caseworker helps youth who are, or were, in foster care to become self-sufficient adults. The ILP program helps gives an opportunity to learn valuable skills necessary to make a successful transition from state or tribal custody to living on your own in the community. ILP caseworker help lead foster youth to self sufficiency by providing multiple services. ILP caseworker determines program eligibility, as well as case noting, monthly reports, weekly life skills groups and monthly one on ones with the youth. ILP caseworker works to empower, encourage, and allow youth to move into adulthood with the knowledge and skills to become responsible and contributing members of their community.Independent Living Skill-Building Program - Provides training and classes to prepare youth to live independently.ILP Discretionary Funds - Flexible funding for various needs to assist youth to achieve goals for transition.Independent Living Housing Subsidy Program - Subsidy to help with housing for up to *30 months (after 12 months, the amount of assistance gradually decreases for the next 18 months, for a total of 30 months support for transitioning to living interdependently in the community).Chafee Housing Program - Provides money for housing, based on need.Chafee Education and Training Grant (ETG) - Provides funds for college or training
