Hans van de Graaff & Associates was established in late 2018 to respond to a growing need for support coordination arising from the roll out of the National Disability Insurance Scheme (NDIS). We provide services across Melbourne's Inner South, Bayside, South East and Peninsula areas.Our team of five staff possess considerable knowledge across disability and mental health, backed by many years of experience working in the industry. We share a passion for making a positive difference to the lives of our participants and supporting them on their NDIS journey.

Responsible for the strategic and service-specific operations of the Bayley House Accommodation Services including Respite Services. Duties include, human resource management, plan implementation, resource allocation, budget management, funding negotiations and program evaluation. A key responsibility is ensuring that Accommodation Services is suitably-staffed with cost-efficient and high quality support options for people with disabilities whose needs can be appropriately met through Bayley House. In addition, the position supports the Chief Executive Officer and Board in the development of the strategic direction and organisational goals of Bayley House, development and implementation of the Quality Framework and meeting legislated compliance requirements. The current key strategic focus is ensuring Bayley House is ready for the implementation of the National Disability Insurance Scheme expected to be rolled out from 1 July 2016.

Responsible for managing and establishing the new headspace Collingwood centre, which is a new service delivery model providing integrated primary mental health services for young people aged 12-25 years. The position has overall leadership and clinical systems responsibility. It includes a strong focus on strengthening and developing existing and new partnerships with Inner North West Melbourne Medicare Local, Inner East Melbourne Medicare Local, City of Yarra, St Vincent's Hospital, Austin Health, Jesuit Social Services, North Richmond Community Health, Mind Australia, Mental Illness Fellowship, Melbourne Clinic as well as others including exploring opportunities in developing the relationship with Collingwood Football Club.

Responsibilities include:Organisational Management (including achievement of organisational objectives, adherenceto relevant legislative requirements and requisite industry standards), Human Resources,Financial Management and Planning & Development.Key achievements: Played key role in writing tenders and significantly growing region between 2004 and 2011 including establishment of Personal Helpers & Mentors demonstration site in the Inner South and currently 2 of the 3 inaugural Victorian Youth Prevention & Recovery Care centres (Sub-Acute residential services ­ PARC's);Fostered and strengthened relationship between Mind and Lantern that paved the way for the eventual merger of those two organisations in April 2012; Developed a strong regional leadership team to support and drive forward the achievement of organisational objectives within the Southern Metropolitan Region;
