Verbinding Support Coordination

Verbinding (formerly known as Hans van de Graaff & Associates) are independent NDIS registered providers of both Specialist Support Coordination (Level 3) and Support Coordination (Level 2), based in the south eastern and bayside suburbs of Melbourne. We have a deep empathy and knowledge of the issues faced by people living with disability and mental health issues, as well as the challenges that their families and carers deal with on a daily basis. Our team are deeply committed to supporting people to live a full life, as well as supporting their families and carers, who are often the sole, unwavering support for people living with a disability or mental health issue. We are passionate about assisting people to maximise the benefits of the NDIS, to live the life they want to lead, as independently as possible.
