iCare Support Coordination

iCare provides support coordination services to NDIS Participants in Geelong & surrounding areas. We offer professional & lived experience, which is our point of difference and our inspiration for quality service provision. We are outcome focused with an ability to walk beside each person’s journey to empower them with choice & control, balanced with capacity building and facilitating change. We take an individual approach and adapt our engagement to meet the needs of the participants we support. Our aim is to help join the dots and connect participants with the right service, at the right time to enable client outcomes and set them up for success. We understand the value of the participants and their families feeling supported, and this is the core of how we operate. Please note, we are currently able to offer services to NDIS Participants that are plan and self managed.
