About Heather Coss
I have been a social worker in community-based health since 2007 after receiving a Master's Degree in Social Work from the University of Wisconsin-Madison. My specialty is working with older adults who want to maintain dignity, quality of life, and as much independence as possible. I focus on a client-centered, strengths-based approach, knowing that clients are experts on their own lives. I began my training at the Geriatric Research and Education Clinical Center at the William S. Middleton VA Hospital, learning about mental health, dementia, and other illnesses common in older age. Additionally, my experience includes working at Care Wisconsin, Inc. for 5 years working in an interdisciplinary team to support and manage the care of dual-eligible older adults and people with physical disabilities. After moving to Santa Fe, New Mexico, I began working as a Care Manager though EGIS Care and Support, LLC. I'm now focused on launching and developing Crossroads Aging Care Professionals, a Care Management group.
