As a Physician Associate in AMU Care of the Elderly, I provide Comprehensive Geriatric Assessment to Acute Medical wards including the same day emergency care (SDEC) Frailty services (via 111, GP, paramedic direct bookings). The role provides enhanced and special services of geriatric medicine consultants throughout the hospital and in delivery of specialist interventions and targeted pathway planning for patients with features of frailty and with complex geriatric needs. The PA team undertake original Consultation, Examination, Social and Collateral History Reviews to formulate clinical management plans under a dedicated team of supervising Geriatric Consultants. We work closely alongside specialty Therapists in Physiotherapy and Occupational Therapy as well as Pharmacy and Social Care providers, to provide a truly comprehensive assessment of the care needs of the Adult. Once assessed the PA team will work alongside the medical team towards the optimum outcome and ensure that all members of the immediate Care team are involved in an empathic and compassionate manner. The Frailty team at GEH deliver forwards planning and complex care planning for the most vulnerable patients in the community. The Care of the Elderly Team provide a first point of call for Frail and Elderly Care as well as a specialist route for assessment of Dementia sufferers or Care Home, or patients who would wish to step up community care to residential or nursing care.

During this role I have worked as a General Practice PA under direct supervision by a senior General Medical Practitioner and Medical Educator. Throughout the year which was split between working in Ambulatory Care - I held my own clinics over 2.5 Sessions. These clinics would include a mix of same day appointments as well as pre-booked and follow-up appointments, telephone follow-ups, results filing, mental health and dementia reviews and QOF specific appointments. I was able to work alongside the GP team and apply knowledge and skills across my role in ACU and GP. The clinics were focused on minor injuries and ailments and this was limited to avoid seeing infants below 5 years (other than for routine 8 week baby checks) which the practice nurses would see. During this role I would consult, examine and perform minor procedures such as vaccinations, wound assessments, ECG's, proctoscopy. From consulting and documenting in the medical records, I was responsible to synthesize clinical management plans semi-independently and to work with the prescribing clinicians to source prescriptions from the On-Call GP's team. I worked closely alongside the Practice Paramedical Practitioner to decide whether home visits were necessary and coordinated evidence of care to enable optimum patient results. I was able to take an active role in the acutely deteriorating patients in the community and enable rapid same day assessment and treatment on site and admission to local hospital centers in Peterborough, Wisbech and Norwich under specific pathways of assessment.

During my role in the Ambulatory Care Unit I was responsible as a front-line clinician to take a clinical history, consider examination and diagnostic findings and to ensure effective evidence-based same day emergency assessment, triage and treatment. These patients were via GP same day urgent referral or from step-down care from A&E or post-discharge follow-up. My role in the department was to facilitate prompt and appropriate wrap-around care to ensure that the liaison with the General Practitioners and senior Hospital Consultant lead care was sufficient and met the needs of the patient as well as the service specifications across the local hospital Trust and community resources. I was the primary author of all patients that I examined and all treatment plans were agreed under the supervision of senior consultants in General, Acute, Renal, Cardiology and Gastroenterological Medicine. This was a fantastic area of learning for me as this was an opportunity to work intensively with many specialties simultaneously and support a unique subset of hospital patients. I worked in support of an Educational environment for all colleagues and patients including Junior Dr's, Nurses and other Allied Professionals in the NHS.
