The key responsibilities whilst in this role is building supportive, trusting relationships with customers and external partners, creating a positive atmosphere. Whilst empowering customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making.Key-working the most complex service-users with needs aligned and dealing with frequentrelapses. Create Risk management and support plans for providing appropriate services based onon-going assessment and reflecting the services and resources available to enable customer to progress in their recovery.Look ahead is an organisation which works with vulnerable people to make choices, achieve goals,and take control of their lives through high quality care, support, and housing. Whilst here I have supported vulnerable adults who had diagnosis of schizophrenia alongside other mental andphysical complex needs. My knowledge and skills progressed extensively within server mentalhealth and facing challenging situations in supporting service users such as mental relapses,homelessness, substance, and alcohol misuse and so on. Having worked within this environment itgave me the confidence to face challenging situations, provide support and access to relevantservices to enable customers to continue their recovery.
